This guide will help you and your colleagues reimagine and transform health in your region. It teaches local leaders how to develop and sustain effective stewardship.

Stewardship is one of three crucial components—along with strategy and financing—that create the conditions for successful systems change. Stewardship occurs when regional stakeholders–leaders, residents, and organizations from all sectors–align around priorities, strategies, and a shared vision for fostering healthy people and thriving communities.

The guide is organized into six sections. First up: an overview of the Pathway for Regions Transforming Health. The Pathway is a visual compass for regional leaders working to move their fragmented health systems toward fully integrated systems. Five more sections describe each of the five key phases along the Pathway. Which phase is your region in?
